In order to better serve you and to create quality programming we need your help. Please fill out this participant form entirely along with a copy of your current health insurance information. This will be kept at the church in a safe place for the purpose of accurate record keeping and so that will only have to do this one time for all trips.
If you have any special needs (allergies, medicines, etc) please write it in on the sheet somewhere.
